  2. Use this form only if you have determined that you cannot use the Software Center.
  3. At the end of the last step, be sure to click the "Finish" button to complete and print a checkout form. (Using the browser menu or toolbar will NOT print the correct form).
  4. Your MVCC Photo ID will be required at the Library and must be updated in order to borrow CDs.
